This course is primarily for those who work within a health & social care setting. Delegates will learn about the development of modern day care, relevant legislation and why person-centred care planning is crucial to ensure the care provision meets the service users individual needs. The course considers a holistic approach which influences & encourages staff to considers a person’s values, along with the core principles that are fundamental to good care planning.

-  Understand who is involved in a Care Plan

-  Understand who is entitled to read Care Plans/Need to know basis

-  Understand how to write with a person centred approach

-  Understand how & when to evaluate Care plans
